What Are You Actually Building? The 3-Year Question
Apr 03, 2026Most owners are solving:
- This lease
- This hire
- This cash flow dip
- This month's margin
- This insurance lag
That's normal. But here's the problem: if all you ever solve is the next 30 days, you drift.
And drifting looks like:
- Over-expanding
- Under-expanding
- Hiring reactively
- Holding back unnecessarily
- Feeling busy but not strategic
The question very few owners ask is: what does this business need to look like in 36 months for me to win? Not survive. Win.
The 36-month filter
Before you sign a lease, before you hire, before you expand, ask:
- How many clinicians do I want in 3 years?
- What revenue target makes this meaningful?
- How much do I want to earn personally?
- How many hours do I want to work?
- Do I want one location or multiple?
If you don't define the destination, every decision feels random.
Without 3-year clarity, three things happen
- You over-expand. You lease too much space too early.
- You under-expand. You stay small because it feels safe.
- You drift. Revenue grows, but so does complexity. Margin fluctuates, but there's no design behind it.
Growth without direction is just movement.
Strategic modeling changes behavior
When you say, "In 3 years, I want a $2M practice with 4 clinicians and 20% margin," decisions suddenly have context.
- That lease? Does it support the 4-clinician plan?
- That hire? Does it move us toward $2M?
- That pricing strategy? Does it align with 20% margin?
You stop reacting. You start architecting.
Operator thinking vs. architect thinking
Operator thinking asks, "How do I make this month work?" Architect thinking asks, "What structure creates the outcome I want?"
Both matter. But if you stay in operator mode too long, the business runs you.
The moment you model 36 months out, you see hiring, pricing, capacity and risk differently. You move from a tactical owner to a strategic builder.
The hard truth
You cannot maximize margin, reduce stress, grow revenue and maintain control without clarity about what you're building.
Expansion without vision creates chaos. Expansion with vision creates enterprise value.
So I'll leave you with this: three years from now, what does winning look like? Define it. Then build toward it.
