Your First Admin: When to Hire and What to Hand Off
Oct 24, 2025There's a moment in every growing practice where you hit the wall.
Revenue is steady, maybe $20K months or more. Patients are coming in. The calendar looks good.
But behind the scenes? You're buried.
- Emails pile up.
- Scheduling eats hours.
- Billing feels like a second job.
And here's the worst part: you know you should be focusing on patient care, growth and profitability, but you're stuck chasing invoices and moving appointment blocks around.
That's when the question hits: "Is it time to hire my first admin?"
Done wrong, an admin is just another expense. Done right, they become the multiplier that frees you to be the CEO of your practice.
Signs you're ready
- You're consistently hitting $20K+ months.
- You're working nights or weekends to keep up.
- You're drowning in $20-an-hour tasks while leaving $200-an-hour decisions on the table.
The mistake? Owners either wait way too long (and burn out) or hire too soon with no clear plan.
The real block is trust, not time
Practice owners are champions at grinding. You hit capacity, so you work harder. You get overwhelmed, so you work longer. And somewhere along the way you convince yourself: "No one can do it like me."
But doing more keeps you stuck. Doing better, with help, sets you free. The real block isn't time. It's trust: in other people, in systems, and in a business that can operate without your fingerprints on every inch of it.
Build your stop-doing list
Write down the top 5 tasks that drain your energy. For example:
- Scheduling
- Ordering supplements
- Chasing invoices
- Email follow-up
- Room flow management
Track your time for a week if you're not sure. The tasks that repeat are delegation gold.
Now put a star next to two of them. Those are the first tasks you hand off. Not "someday." Not "once things calm down."
Ask yourself:
- What tasks make me resent my business?
- What do I keep doing because it's "easier to just do it myself"?
- Which task would immediately give me more mental space if I never touched it again?
Start small, with systems
A part-time or virtual admin for 5 to 10 hours a week can clear the logjam. But only if you set them up with systems, not "just figure it out."
Give them a clear playbook: scheduling protocols, billing processes, email templates. Every task you take off your plate should come with rules, not guesswork. Every minute you spend training saves ten later.
The payoff
- You reclaim hours every week.
- Patients get faster, smoother service.
- You finally have space to think like a CEO.
You're not giving up control. You're buying back capacity. That's the shift that turns a $30K-a-month practice into a $50K-a-month practice without doubling your hours.
Hiring your first admin is not a cost. It's an investment in freedom.
